If a child needs meal accommodations due to an allergy or other dietary considerations, the Request for Meal Accommodations form must be completed and signed by a medical professional as designated on the form. Meal accommodations cannot be provided without the proper documentation on file. Please return a completed form to the school nurse. The Request for Meal Accommodations form can be downloaded here:
