Registration Checklist
If you are new to the Blaine County School District, you will need to provide the following documents:
1. Original STATE CERTIFIED birth certificate. NOT a hospital certificate.
2. Immunization record with: click to view Immunization Requirements.
Vaccinations can be updated by your physician or at South Central District Health, 117 E. Ash in Bellevue, 788-4335 (appointment needed).
3. Proof of residency (see Residency Policy and Residency Procedure) with a PHYSICAL address in Blaine County:
4. TWO (2) CURRENT UTILITY BILLS, written proof of utility service, or lease agreement indicating utilities paid with lease; any of which include name and physical address in Blaine County; AND one (1) of the following additional documents:
-VALID IDAHO DRIVER'S LICENSE OR STATE ISSUED I.D. CARD indicating physical address in Blaine County.
-A PROPERTY TAX BILL indicating physical address in Blaine County.
-A CURRENT LEASE AGREEMENT indicating physical address in Blaine County.
-A NOTARIZED AFFIDAVIT SIGNED AND SWORN FROM CURRENT LANDLORD OR HOMEOWNER of physical address in Blaine County.
-IDAHO TAX RETURN indicating physical address in Blaine County.

Immunization
* 39-4802. EXEMPTIONS. (1) Any minor child whose parent or guardian has submitted to school officials a certificate signed by a physician licensed by the state board of medicine stating that the physical condition of the child is such that all or any of the required immunizations would endanger the life or health of the child shall be exempt from the provisions of this chapter. (2) Any minor child whose parent or guardian has submitted a signed statement to school officials stating their objections on religious or other grounds shall be exempt from the provisions of this chapter.

Neighborhood Schools, STEAM, Dual Immersion and Transportation
Child Using Transportation Services: The selection of a neighborhood school is very important and should be made prior to registration. Having the daycare address for your student’s pick-up/drop-off location ready at registration helps assure your student's transportation schedule.
EHSS STEAM School and Alturas Dual Immersion School Lottery: Families interested in entering their child into the lottery for STEAM education at Ernest Hemingway or Dual Immersion education at Alturas Elementary may need to participate in the school lottery system. This includes families with students already in the program. Please reach out to the schools for more information.
